Liposuction/Liposculpture
Exercise and diet will make you healthy, muscular, and lean. However, it does not always remove those stubborn LFD’s (localized fat deposits). Every individual’s fat distribution and body shape is genetically determined. What this means is that often, no amount of diet and exercise will remove these stubborn fat deposits. Liposuction removes fat and fat cells thus sculpting the body and doing what diet and exercise alone cannot. Liposuction is one of the most common cosmetic procedures performed in the United States today. Lipoplasty, commonly known as liposuction, is a common surgical procedure that removes deposits of excess fat from specific areas of the body, face or neck. Dr. Heidari uses the latest techniques to slim your hips, thighs, flatten your tummy, shape your calves and ankles or eliminate your double chin.
The ideal candidates for Liposuction are:
1. Men and women who are within 10-15% of their ideal body weight and want to improve their body shape and contours.
2. People with fat deposits that are “trouble areas” where exercise and diet do not seem to have any effect.
3. People with good elasticity in their skin, so when the fat is removed, they will achieve a smooth appearance.
The areas that liposuction can have the greatest effect on are the abdomen and waist, thighs, the arms, back chin, neck, hips, buttocks, thigh ankles, calves, and inner knees. Patients of any age can undergo the procedure, but those who have greater skin elasticity will achieve the smoothest end result.
Dr. Heidari uses the tumescent technique with micro-cannulas. With the tumescent technique, the fat is pre-injected with a solution of salt water, local anesthetic, and small amounts of adrenaline.
During the procedure, one or more incisions are made near the area to be treated. When possible, the incisions are made within the fold or natural creases in the skin. Dr. Heidari will then insert a small hollow tube, called a cannula, through the incision where he will then penetrate the underlying fat. The cannula is connected to a flexible tubing and suction pump. He then moves the cannula back and forth through the fat, vacuuming the fat out through the tube in a very precise manner. The results are a beautifully sculpted and smooth body.
Liposuction is very common with thousands of patients undergoing the procedure each year. In rare cases the procedure has caused severe trauma, particularly when multiple or very extensive areas are suctioned at one time. Dr. Heidari carefully examines each liposuction candidate to maximize patient safety. Other possible complications include fluid accumulation and injury to the skin. Sometimes asymmetry or unevenness may require further treatment.
Recovery
After liposuction, you are in a binder for 3-5 weeks depending upon the areas treated. You can take a shower in about 3 days. Pain is very manageable and usually, you can return to work within 3-5 days with their garments on and return to regular exercising in about 4-5 weeks.

Tummy tuck (Abdominoplasty)
Abdominoplasty (tummy tuck) is a surgical procedure in which excess skin with stretch marks and fat are removed from the lower abdomen and the muscles of the abdomen wall are tightened. This procedure can drastically reduce the appearance of a protruding abdomen. The procedure is particularly helpful to women who, through multiple pregnancies, have stretched their abdominal skin, resulting to loss of muscle tone and stretch marks. Dr. Heidari often performs tummy tucks in conjunction with liposuction to create a more attractive overall appearance of the body.
The ideal candidates for an abdominoplasty are men or women in good health and relatively good shape, but are bothered by a large fat deposit or loose abdominal skin with stretch marks that would not respond to exercise.
Tummy tuck is a major operation. Some of the risks of tummy tucks include infection and blood clots. These can be treated easily and are rare. Dr. Heidari carefully evaluates each patient pre-operatively and takes every necessary step to reduce the operative risks for you. Tummy tucks have been known to leave scarring. Dr. Heidari carefully designs the scar to be hidden with the under garment, and uses the latest laser technology available to significantly reduce the appearance of the scar. He will explain fully as to what to expect post-operatively for the fastest recovery and best cosmetic results.
Recovery
After the tummy tuck, you are in a binder for about 5 weeks. The overall recovery takes a little more than liposuction. You can take a shower in about 5-7 days after drains are removed. Drains are placed under the surgical area to remove excess fluid and blood and can be easily managed by the you or your caretaker. Pain is very manageable through a pain pump and usually, patients ambulate the first night after surgery. You can return to work within 3 weeks with your garments on and return to regular exercising in about 5-6 weeks.

Post Massive Weight Loss plastic surgery
Bariatric surgery can be highly successful in helping morbidly obese patients lose weight. After the procedure, patients are often able to enjoy, not only significant weight loss, but also greater mobility, relief of symptoms and a lower risk of weight-related diseases. This further boosts their self-confidence. One unfortunate result is that the skin hangs loose over the slimmer body.Dr. Heidari can create the more attractive body contours that you deserve by tightening, lifting, and contouring sagging skin for a more attractive appearance.During consultation, Dr. Heidari will evaluate your contouring needs that may include your arms, breasts, abdomen, buttocks, thighs or other areas.
Either with increasing age or significant weight loss, there is often an excess of body skin and fatty tissue. In the area of your back and belly (the trunk), cases of mild excess may be treated appropriately by procedures such as a tummy tuck or liposuction. However, in more significant excess situations, especially where the excess is circumferential (circular) in nature involving your belly, hips, back, buttocks, and outer thighs, a more extensive procedure is required. This procedure is called “Belt Lipectomy” and is like a face-lift for the trunk.
Other names for this procedure include body lift, lower body lift and circumferential panniculectomy.
Ideal candidates for this post-bariatric plastic surgery are:
1. Patients that have lost their expected weight after their bariatric surgery and kept it off for at least 6 months
2. Non-smokers
3. Patients with no major heart or lung disease that can tolerate general anesthesia
4. Post-bariatric plastic surgery sometimes requires a pre-surgical psychological counseling.
As with any surgical procedure there are possible risks and complications such as infection, bleeding and seroma (fluid accumulation) in the surgical areas of the back. This is usually treated by repeated needle aspirations. Wound separation is another possible risk early after surgery. Careful detailed instructions and close monitoring by Dr. Heidari before, during and after surgery helps minimize these risks. Blood clot formation in the legs after surgery can lead to pulmonary embolus, which is a blood clot that travels to the lungs. Careful pre-operative history of the patient as well as precautions taken during surgery, such as leg compressions or low dose blood thinners, helps significantly decrease these risks. All post-bariatric plastic surgery patients are expected to start ambulating as soon as possible after surgery.
Other areas to be reshaped after massive weight loss include:
1. Removing and contouring excess breast tissues
2. Removing excess skin and fat and tightening upper arms ( brachioplasty)
3. Removing excess skin and fat and tightening the inner thighs ( thigh lift)
Recovery
All patients are required to stay in the hospital for a minimum of two nights. Patients have a number of drains, (plastic tubes left under the skin and fat) to collect tissue fluid and blood, left in place for 1 to 2 weeks. The patients are expected to get up and walk the first night of surgery and their activity is slowly advanced over the next few days. They have to walk in a slightly bent position for a week, at which point they are allowed to start maneuvers to straighten up. Most patients are back to their daily activities in 4-6 weeks and return to regular exercising in about 6-8 weeks.

Arm Lift (Brachioplasty)
It is unfortunate that the skin will often hang under the arms, particularly after a patient loses a great deal of weight and has worked diligently to do so. The only way to remove this excess skin is through surgery. Arm Lift (brachioplasty) is performed to eliminate the excess fat and skin that sometimes occurs under your arms as a result of normal aging or following weight loss. While you may lose additional weight through a brachioplasty, this procedure is not meant as a weight-loss tool, but as a way to sculpt the upper arms.
Dr. Heidari places the scar of the upper arm toward the back for a more natural cosmetic result. Excess fat is removed, the skin is lifted and arm ligaments are tightened. Sometimes, liposuction is combined with the procedure for fat removal. Once the incisions are closed, dressing is placed around the arms and a light compression garment may be used for a few days. In some cases, drains are inserted for a day or two. Arm lift is often performed along with other body contouring procedures, such as a body lift, thigh lift, or abdominoplasty.
Recovery
After an arm lift procedure, you remain in soft dressing for 3-4 days and then usually wear a compression garment for 2-3 weeks. Pain is very tolerable and daily activities start at about 2 weeks, exercising around 5-6 weeks.

Thigh lift
It is unfortunate that extra, sagging skin often remains after a patient has worked very hard to lose a great deal of weight. When thighs are disproportionate to the rest of the leg and/or the buttocks, one solution is to remove excess skin and fat with a thigh lift. Liposuction is often included in thigh lifts for better contouring. In the case of a post-bariatric weight loss patient, large amounts of skin excess can be removed to create a more attractive shape.
Ideal Candidates for a Thigh Lift are:
1. Post bariatric healthy adults who have had significant weight loss and had kept it off for at least 6 months
2. Patients with combination of excess fat and loose skin of the inner thighs
Dr. Heidari performs thigh lifts on an outpatient basis using general anesthesia. An incision is made in the crease where the upper thigh meets the buttocks or in the groin area near the hip. The skin is carefully separated from the underlying muscle and tissue. Your excess skin is removed. If needed, liposuction may be used to remove excess fat. The remaining skin is stretched and tightened over the area and sutured closed. The surgery takes between 2 and 3 hours. In cases of extreme skin excess, a vertical incision of the inner thigh is necessary for optimal results. Drainage tubes may be inserted to help remove excess fluid for the first two days. Thigh lift is often combined with a brachioplasty to rid the upper arms of sagging skin, and/or an abdominoplasty.
Recovery
After a thigh lift, a light dressing will be applied following the surgery. A light compression garment is then worn for 3-4 days after surgery, and up to 3 weeks after. Swelling and bruising are common and subside over a few weeks. Stitches are removed after 7 to 10 days. Most patients are back to work in about 1 week and resume most normal activity within 3 weeks, exercising around 5-6 weeks.

Male breast Reduction
Though few people talk about it, gynecomastia is fairly common. This affects an estimated 40-60% of men. Gynecomastia is a medical term that comes from the Greek and literally means “woman-like breasts.” It can affect one breast or both. Although certain drugs and medical problems are linked to gynecomastia, there is no known cause in the majority of cases. For men who are self-conscious about their breasts, male breast reduction surgery can help. The procedure removes fat and glandular tissue from the breasts and, in extreme cases, excess skin may also be removed. The result is a chest that is flatter, firmer, and with a more athletic look.
Ideal candidates for male breast reduction surgery:
1- Healthy males with good elasticity of their skin
2- Teen-aged males with normal blood hormones and no liver dysfunction
3- Males who do not drink excessive alcohol, take anabolic steroid or smoke marijuana
Gynecomastia occurs frequently among teen males because of hormone fluctuations, but naturally resolves itself over time in most cases. When gynecomastia does not resolve by itself, Dr. Heidari performs breast reduction on teen-aged boys, as large breasts on a teenager can be embarrassing for them and create psychological and social problems. Surgery is discouraged for men who are overweight unless they have first attempted to correct the problem with exercise or weight loss. For some men the condition is hereditary; for others, medical conditions such as cancer and impaired liver function contribute to the problem. Today, men with fatty deposits that create raised breast areas can take advantage of a surgical technique to remove the fatty deposits and excess tissue. Dr. Heidari performs male breast reduction surgery on an outpatient basis using either a light general anesthetic or local anesthesia. This depends on the amount of tissues to be removed. A small incision is made at the bottom of the areola (the brown area encircling the nipple). Dr. Heidari then removes any excessive glandular tissue and skin. He often includes liposuction in the procedure for optimal contouring of the chest. The surgery generally takes about 2 hours. Sometimes, drainage tubes are used for the first two to three days to help remove excess fluid. Your chest will likely be wrapped to help reduce swelling. Some swelling, bruising and numbness are normal and will subside over time. Stitches are removed in 1 to 2 weeks after the surgery.
Recovery:
You can return to work within 4 to 7 days. An elastic pressure garment is worn for 3 to 6 weeks, and sports and strenuous activity are limited for 3 to 4 weeks after the surgery. Patients should avoid direct sun exposure or salon tanning for 6 months.
